Who will make healthcare decisions for you if you are unable and are they equipped to honour your wishes? This is important information for everyone, at any age.
- Learn who is your substitute decision maker (everyone in Ontario has one) and if you are someone’s substitute decision maker
- Learn how to ensure they understand your goals, values, and wishes, so you feel they are able to speak for you in the event you are unable
- Learn what you need to know and understand if you are to speak on the behalf of loved ones as a substitute decision maker
