Evergreen Hospice – Advance Care Planning and Substitute Decision Makers

Who will make healthcare decisions for you when you are unable and are they equipped to honour your wishes?

Please join us on Zoom for an Advance Care Planning and Substitute Decision Maker Information Session. In this session we will:
1) Determine who your substitute decision maker is (everyone in Ontario has one)
2) Learn how to ensure they understand your goals, values, and wishes so you feel they are able to speak for you in the event you are unable
3) Discuss what you need to know and understand if you are to speak on the behalf of loved ones as a substitute decision maker.

Please share with anyone who you feel may be interested in attending. This is open to clients, volunteers, staff, and community members. This is important and helpful information for anyone at any age.
